Description:
Providing safe transportation to COVID-19 vaccine appointments. COVID-19 safety protocols are in place to ensure passenger and driver safety.
Wearing a mask is mandatory during the trip.
You must already have a vaccine appointment booked in order to request transportation.
After your request is sent, someone will contact you directly to confirm your pick up time.

Eligibility Criteria
Criteria:
- Client has a vaccine appointment booked (*Providers will do their best to accommodate rides to walk-in and pop-up vaccination clinics if they have driver availability)
- Client is a person with a disability, including physical, mental, developmental, learning, mobility, vision, or hearing impairment
- Client has limited or no access to safe and accessible transportation or face barriers to getting to/from a COVID-19 vaccine appointment
