Description:
The Diabetes Education Team works together with the client, their doctor, and other health care professionals to help manage the client's diabetes. Goal is to help the client gain knowledge about diabetes, become an advocate for their own health, and give them the tools and skills to take control of their blood sugars.
Self-referrals are welcome and family doctor is not needed to attend the programs.

Eligibility Criteria
Criteria:
- Adults (18+) with Type 2 Diabetes, Type 1 Diabetes, and those with Pre-Diabetes and Gestational Diabetes.
- Person completing the form is the client or has gotten permission from the client to complete the form.
