If you are a Health Care Provider, you can submit a referral to our program to assist with the varying needs of your client in the community.

Community Paramedics can provide the following:
- Responding to changing circumstances or medical conditions, and connecting to the right health care provider/social services to avoid escalation and crisis.
- Treatment of varying disease pathologies under our medical directives to help streamline and prevent the escalation of acute and chronic conditions.
- 24/7 non-emergency contact: Members of our team will help assess the current medical presentation and coordinate a home visit with one of our Community Paramedics and/or, within scope, determine further escalation.
- Clinical Interventions such as diagnostics such as specimen collections for both point-of-care testing or transport to a laboratory for processing. We are also able to complete 12 and 15 lead ECGs, digital recording of lung sounds, vital signs, wound care, coordination of community services, and observational/focused assessments in the comfort of your client’s home.
- Immunization, vaccination, and other injections (e.g. shingrix, B12, RSV, pneumovax).
- CPRPM - remote patient monitoring of blood pressure, heart rate, oxygen saturation, blood glucose, and weight.
- Assisting our community partners with various medical procedures at home (e.g. difficult IV starts, medication review, if requested)
- Other controlled medical procedures and treatments at home under appropriate medical supervision
- Palliative care; acute treatment and support to clients and their community palliative care teams.
