Description:

Enhanced In-Home Transitional Services (EHTS) is a community-based support service for older adults and seniors who are designated ALC at Toronto-area hospitals and who are able to be safely transferred home with access to personal care support.



The service provides case management and personal support services such as bathing, medication support, incontinence care and light housekeeping.



Are these eligibility requirements met?

Client has a confirmed ALC status from hospital
Client has confirmed discharge (home) destination and address
Home has all equipment necessary for client to live there safely
Client lives between Yonge St and the Humber River, and between Lawrence Ave and Lake Ontario
Will be able to manage with HCCSS services after 90 days
Minimum Age: 55 yrs
