The Case Management Team provides assistance and supportive counselling to seniors who have identified the need for assistance in accessing supports and services while living in the community.. In-home and in-office assessments are provided to plan and coordinate services to meet the needs of the individual.

The patient must have mental/cognitive/physical health issues, and be in need of services, supports or resources to manage their day-to-day responsibilities and live in the Geri-Hub catchment area (South of Eglinton Avenue, North of Lake Ontario, West of Bayview Avenue, and East of the Humber River
